Volunteer Acknowledgement, Age Policy, & Liability Waiver

Age Requirement & Supervision Policy: The minimum age for independent volunteering with the Preconception Health Awareness Foundation is 18 years. Applicants under the age of 18 are welcome to support our mission but must have explicit parental/guardian consent and are required to be accompanied by a parent, guardian, or a designated supervising adult during all in-person foundation activities and community outreach events.

Waiver & Release: By submitting this application, I understand and agree that services are provided on a strictly volunteer basis, without anticipation of financial compensation or future employment.

I hereby release, waive, and hold harmless the Preconception Health Awareness Foundation, its directors, officers, employees, and partners from any and all liability, claims, or demands for personal injury, sickness, or death, as well as property damage and expenses, of any nature, occurring during my participation in foundation activities, events, or field projects.

I also understand that I am responsible for maintaining strict confidentiality regarding any sensitive organizational, donor, or beneficiary data I may access during my time as a volunteer.
